Durham County Council (the Council) are seeking submissions from suitably experienced and skilled organisations to provide the specified services for the Health, Wellbeing and Night Hub. This includes the Safe Night Hub, Integrated Support Hub and dispersed properties. The main service should be based in one premises which is able to deliver the wrap around support required. The contract will also include the provision of 10 dispersed supported accommodation properties to be used as a step down pathway from the Night Hub. The Health, Wellbeing and Night Hub aims to deliver a preventative, integrated response for adults at risk of homelessness, rough sleeping, or poor health outcomes, particularly those discharged from hospital or unable to safely return to accommodation. PCH helps councils to improve the way they deliver adult social care and public health services and helps Government understand the challenges faced by the sector. The programme is a trusted network for developing and sharing best practice, developing tools and techniques, providing support, and building connections. It is offered to councils without charge. Reading’s CQC assessment was published 10th October 2025. CQC feedback highlighted • delays in assessments and reviews, • inconsistent carers’ assessments • long delays in direct payments This project aims to address root causes, supporting sustainable service delivery and improvement outcomes for local people and carers. In order to address the areas highlighted by the CQC the Council wishes to manage demand, reduce waiting times and implement sustainable system change in Adult Social Care. The aim is to establish a robust, evidence led prevention framework for Adult Social Care, with a focus on improved outcomes for residents, demand reduction, delayed escalation into statutory services and reduced pressure on ASC front door including the Advice and Wellbeing Hub and Hospital pathways.

The aim of the Service Specification is to ensure the provision of high quality, outcome-focused services which are safe and that meet the quality outcomes and safeguarding responsibilities of the Council. This will involve working to achieve the agreed outcomes as defined by the Service User, through a person-centred planning approach that puts Service Users and their Carers at the forefront of all decision making. The services that this will be achieved through include a 24/7 care staffing presence to respond to unplanned care requests and help engage with the delivery of activities (referred to as the 24/7 Core Wellbeing Service) as well as through the delivery of Planned Care and Support as requested by the individuals residing in the apartments to which the Council holds Nomination RightsWhile City Development manage the building phase of the scheme, responsibility for securing an onsite care provider is managed by the Integrated Commissioning Team in the Adults and Health Directorate. Under this approach, the service specification stipulates the nature of the service and the expectations of Leeds Adults & Health (Leeds A&H) in terms of delivery. The service specification does not offer any guaranteed hours, instead it states the potential opportunity for care hours from nominated tenants which would then be paid for through direct payments, managed budgets or from an individual’s own finances. The service to be procured consists of two components: an onsite, core service providing 24/7 cover to offer peace of mind, which includes responding to alarm calls, and; the provision of statutory planned care and support for those individuals who wish to purchase this service from the onsite provider, as opposed to any other care provider options. 26022 Support for Children Living in DA Households

The aims of this contract are to help provide stability and support for children aged 5 – 18 who are experiencing domestic abuse between their parents and/or carers. The aim is to support children who are currently living in the household or who have recently (3 months) vacated the household in which DA was occurring. The support offered should take a child centred approach, giving a voice to the young person (YP), and should cover a wide range of issues such as emotional impact, coping mechanisms and health relationship work. This is to be delivered in a series of bespoke 1:2:1 intervention which have been tailored in a way to suit the needs of the YP. The second aspect of the support offered is to parents and carers in helping them understand the impacts of DA upon their YP and how they might need to change or respond differently to the needs of their YP. Lastly, where direct support isn’t possible, the provider will be required to work with relevant DA professionals to ensure additional support routes may be offered. Examples of this are through training, sharing of resources or additional external support structures. The successful provider will need to work with fellow support professionals to ensure that the young persons needs are being met and their quality of life is improving. Said provider will also be required to act as an advocate for the YP where appropriate through engagement with further professionals. Please note that the service will not be supporting YP where there is domestic abuse within their own intimate/personal relationships or where there is direct sexual abuse/violence.

The outreach service will deliver interventions to reduce harm around substance use and associated risky behaviours with a target audience of 16–25-year-olds. The framework is designed to support strategic commissioning approaches that improve outcomes for children, strengthen sufficiency and market shaping, and deliver improved value for money. The model aligns with regional ambitions, including the development of Regional Care Cooperatives, and will facilitate call‑offs at both regional and local levels. The procurement aims to foster long‑term, collaborative relationships between Commissioners and Providers. It seeks to address the differing perspectives, risks, and challenges that influence the delivery of high‑quality homes for Children and Young People. The objective is to secure local, stable, and supportive homes that are embedded within a wider system of wrap‑around provision, enabling improved long‑term outcomes. This represents a shift away from the traditional purchase of individual placements through spot purchasing or standard frameworks. Instead, the framework focuses on commissioning whole homes, ensuring that the home, its staff, and the children living there are supported by coordinated local services. Contracting Authorities will be able to undertake a Competitive Selection Process at call‑off stage, based on their specific requirements. Whereby Warwickshire residents aged 40-74 without any predefined existing health conditions can access an NHS Health Check every 5 years. The NHS Health Check (NHSHC) programme screens 40-74 year olds for risk of health problems, such as, heart disease, diabetes, kidney disease and stroke. It achieves this by assessing the top seven risk factors driving the burden of noncommunicable disease in England, and by providing individuals with behavioural support and, where appropriate, medical intervention. Local authorities are required to provide health checks as this is a mandated service under the Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch Representatives) Regulations 2013 Act.1 Local Authorities have a statutory duty to provide, or make arrangements to secure the provision of, NHSHCs to be offered to eligible persons in its area once every 5 years, and to secure the provision of NHSHCs. The local authority should also secure continuous improvement in the percentage of eligible persons in its area participating in the NHSHCs. The need for the service has also been informed by service reviews, engagement with members of Service provision will be free for all service users.
